All caregivers must clock in/clock out using the Vesta Electronic Visit Verification (EVV) system. Employers must approve time depending on which option (1, 2, 3) they selected on Form 1722. Please note, option 3 is the default option if the employer did not submit Form 1722 to Consumer Direct Care Network Texas (CDTX).


Form 1722 Options

  • Option 1:
    • Caregiver: Clocks in/out using an approved Vesta EVV method. Please do not send in a timesheet. No timesheet is needed as the employer can make all corrections.
    • Employer: Corrects errors in the Vesta CDV Portal and approves time in the Vesta CDV Portal.
  • Option 2:
    • Caregiver: Clocks in/out using an approved Vesta EVV method.
    • Employer: Approves time if accurate. Submits an EVV Attestation of Service Delivery/EVV Visit Maintenance Form to CDTX if there is an error.
    • CDTX:  Corrects error in Vesta system and submits back to employer for approval in the Vesta CDV Portal.
  • Option 3:
    • Caregiver: Clocks in/out using an approved Vesta EVV method.
    • Employer: Submits an EVV Attestation of Service Delivery/EVV Visit Maintenance Form that matches the EVV shifts that were claimed by the caregiver in the Vesta system.
    • CDTX: Verifies that the time on the EVV Attestation of Service Delivery/EVV Visit Maintenance Form matches the EVV shifts in Vesta, corrects errors as needed, and approves the attested time in the Vesta system.

EVV Attestation of Service Delivery/EVV Visit Maintenance Form

The EVV Attestation of Service Delivery/EVV Visit Maintenance Form must be submitted to CDTX by Monday at midnight for the previous week. Please refer to the 2021 payroll calendar. Late submissions will result in late pay. Please submit the form to CDTX by email or fax.


Important Reminders

CDS employers are responsible for providing their caregivers with the information and training necessary to use the Vesta EVV mobile app, landline option, or alternative device.

CDTX selected Vesta as our EVV vendor. Vesta or CDTX will email employers their username and password. Employers who have not received their login information should email InfoCDTX@ConsumerDirectCare.com immediately. Please allow at least two business days for a response.
